How to request a transcript from Augustana College:

Augustana College is pleased to provide this service free of charge.

Fill out the Transcript Request form

E-mail requests must include a scanned request including your signature.

If you are unable to use the above form, please send a letter or fax that must include the following information:

1. Your name(s) while you were at Augustana.
2. Your birth date, social security and/or student identification numbers.
3. The approximate dates of attendance.
4. Where you want the transcript(s) sent.
5. Your written signature must be on the letter or fax.
