How to request a transcript from Augustana College:

E-mail requests must include a scanned request including your signature.

The letter or fax must include the following information:

1. Your name(s) while you were at Augustana.
2. Your birth date, social security and/or student identification numbers.
3. The approximate dates of attendance.
4. Where you want the transcript(s) sent.
5. Your written signature must be on the letter or fax.

Augustana College is pleased to provide this service free of charge.
