Health Insurance
U.S. Residents
Augustana recommends that all full-time students have health insurance or other medical plan coverage that complies with the Affordable Care Act. Full-time students (or part-time students who have paid the full activity fee) may visit the Health Services on campus or at our designated Sanford Clinic with no charge for office visits. Students will need to supply their insurance card for lab work, shots and procedures that are not included in the basic office visit charge.
International Students
Augustana College requires all international students to maintain medical insurance that provides coverage in the United States and meets certain minimum benefit requirements. Augustana will automatically enroll all international students in Augustana's International Student Accident and Sickness Plan. The insurance premium will be added to the student's bill.
If a student wishes to have the plan waived, proof of coverage must be provided that specifies that the alternate policy provides benefits at least equal to those required by Augustana College.
Waiver Instructions: Ask your insurance company representative to complete the waiver form and return it to Augustana College. A waiver must be completed every academic year, even if the student was enrolled and waived in the prior academic year. (See below to download the insurance plan and waiver form.)
(International insurance plan brochure and waivers for the 13-14 academic year are not available yet.)
Student Health Records
Students are required to submit a health record form (Adobe Acrobat .pdf file, 360 KB) verifying a recent physical examination and a record of required immunizations before enrolling in their first academic term. New students must provide health care provider-documented immunization status for measles-mumps-rubella (MMR) and vaccination for bacterial meningitis.
