Internship Forms

The following are available as fillable PDF forms:

• Declaration of Intent
• Course Proposal
• Evaluation: Intern self evaluation
• Evaluation: Site Supervisor evaluation of intern

Instructions for use:

• Students:
  1. Select the appropriate form (Declaration of Intent, Course Proposal, or Self Evaluation) from the list above
  2. The form will open in a new browser window
  3. Complete the form using Adobe Acrobat® or Adobe Reader® v6 or higher
  4. Print the form using the PRINTFORM button located on the form
  5. Sign and date the form
  6. Finalize the form:
  • Declaration of Intent: Obtain required signature and deliver to the Registrar's Office
  • Course Proposal: After the Internship Advisor has completed the section on assessment and grading, and has signed the form, obtain Academic Advisor's signature and deliver to the Registrar's Office
  • Self Evaluation: Deliver to Internship Advisor
• Site Supervisors:
  1. Select the Site Supervisor Evaluation form from the list above
  2. The form will open in a new browser window
  3. Complete the form using Adobe Acrobat® or Adobe Reader® v6 or higher
  4. Print the form using the PRINTFORM button located on the form
  5. Sign and date the form
  6. Return to the Augustana faculty internship advisor during the scheduled exit interview
