Student Teacher Self-Evaluation Form

 

The purpose of this exercise is to help you evaluate your growth at about mid-point in the student teaching placement.  Discuss the results with your college supervisor.  After the conference, take this evaluation sheet with you and place it in your portfolio.  Evaluate yourself using the following five-point scale:

 

1) unsatisfactory

2) below average

3) average

4) above average

5) outstanding
